Patient Responsibilities
As a patient of Middle Tennessee Bone and Joint Clinic, you are responsible:
- For providing information about past illnesses, medications, and other health-related matters.
- For requesting additional information or clarification if you do not fully understand thecurrent information or instructions.
- For informing your physician if you anticipate problems in following the treatment that is prescribed.
- For being considerate of and making reasonable accommodations to the needs of the physicians, staff, and other patients.
- For being respectful of the property of other patients and the facilities.
For providing necessary insurance information and working with the clinic as needed to make payment arrangements.

For realizing the impact that your lifestyle has on your personal health because your health can only be influenced so much by the doctor’s prescribed plan of treatment.

For the outcome if you refuse the treatment that the doctor prescribes to you.
