Office Hours
Monday-Friday, 8:00 AM to 5:00 PM
Initial Appointment
Please arrive 15 minutes prior to your scheduled appointment time to allow sufficient time to complete all necessary paperwork.
Insurance Co-Pays
Insurance co-pays are expected at the time of service and can not be waived. We accept cash, check or credit cards.
Referrals
If your insurance company requires a referral from your primary care physician, you must provide the Clinic with all the necessary referral information and documents. It is the policy of our Clinic to receive referral by mail or in person only. We will no longer accept faxed referrals as it is the responsibility of the patient to bring this referral at the time of their office visit. If the patient does not have a referral and is unable to get one, that appointment may be rescheduled. Click here for a more complete list of patient responsibilities.
Appointment Cancellations
Appointments should be canceled within 24 hours to allow other patient an opportunity to be seen. If you need to reschedule or cancel an appointment, please call 931.388.4276.

Insurance Changes
Please notify the Clinic of any insurance changes. A photocopy of the patient's current insurance card will be kept in the chart to ensure prompt and accurate filing.
Patient Information Changes
Please notify the Clinic of any personal record changes such as name, address, phone number. This will allow us to keep our records current. All patients will be asked to complete new paperwork once a year.

Medical Records/X-Ray Requests
All medical records and x-ray are the property of Mid-Tennessee Bone & Joint Clinic. Records will be transferred to another physician or medical facility with the completion of the appropriate authorization forms. There could possibly be a charge for copying Medical Records. There is a charge of $10/per x-ray copy.
